What the FD-258 is

The FD-258 is the FBI's Applicant Fingerprint Form — the standard card with boxes for each rolled finger, plus flat impressions. Agencies across employment, licensing, immigration and firearms applications still ask for it.

Plenty of places still roll these in ink. We don't. We capture electronically and print the FD-258 from that capture, which means the ridge detail is checked for quality before anything is committed to card stock. Same card the agency expects, far less risk of rejection.

Who asks for FD-258 cards

• ATF paper applications — Form 1 or Form 4 filed on paper rather than through eForms. Cards go in duplicate to the NFA Division within ten business days of filing.
• FBI Identity History Summary requests.
• State licensing boards, employers and volunteer organisations.
• Out-of-state applications where you must mail cards to another jurisdiction.

What's included

Two FD-258 cards come with the $50 fingerprinting fee. Additional cards are $5 each — worth taking if there's any chance you'll need a spare, because a second visit costs far more than five dollars.

What to bring

• Valid government photo ID.
• Any card or instructions the agency sent you. Some agencies issue pre-printed cards with an ORI number already filled in — bring those and we'll print onto them where possible.
• The exact spelling and details that will appear on your application.
